MAYESVILLE POLICE ATHLETIC LEAGUE CONTRACT

I,_____________________________ must appear for all scheduled games or notify Major McCombs directly---leaving a message is not acceptable unless I receive acknowledgement.  If notification is not given, I will pay a fee of $1.00 for snacks, weekdays, and $3.00 for Saturdays.  I will not play until outstanding fees are paid. I will be responsible for checking the schedule, which posted on the internet, town hall, the station and the community center.

In return, I will be afforded a safe gang free environment.  Coaches, equipment and a location for basketball are free.  Refreshments will be provided.  There will be no cost as long as I fully participate.

I agree to make transportation arrangements for my child to get to the community center on Pringle St. to the left of the Mayesville Institute. I understand that there must be accountability to ensure a quality experience for the children and I agree with the fee schedule.
